How to Become

a Patient

Open Door Health Center serves uninsured poor individuals in Miami-Dade County.

To become a patient, the interested individuals must:
  1. Complete an application obtained at the Open Door Health Center or download here.

  2. Follow the instructions on the application completely. This includes meeting with our on site Eligibility Worker who will assist the individuals in applying for services (new patients and yearly eligibility renewal).

  3. Show proof of household income.  This is required because we follow the Federal Poverty Guidelines for eligibility.
The clinic follows these procedures:
  1. We do not discriminate based on race, age, gender, religion, ethnicity, country of origin or sexual orientation.

  2. Our services are offered on a temporary basis until the individual(s) or families can obtain other services or can afford health insurance.

  3. We review eligibility criteria every year minimum.
